1、現(xiàn)場安全文明施工措施費費率各建筑業(yè)施工企業(yè)、招投標代理、造價咨詢?yōu)榍袑嵓訌娢沂薪ㄔO工程安全生產、文明施工管理水平，保證現(xiàn)場安全文明施 工措施費落實到位和?？顚Ｓ?，根據(jù)江蘇省建設廳關于印發(fā)江蘇省建設工程現(xiàn) 場安全文明施工措施費計價管理辦法的通知 （ 蘇建價2005 349號） 、蘇州市建設局關于轉發(fā)江蘇省建設工程現(xiàn)場安全文明施工措施費計價管理辦法的通 知（蘇建價 200524 號） 文件精神要求，結合我市實際情況，提出如下實施意 見，請一并貫徹執(zhí)行 .第一條 凡張家港市范圍內各類新建、擴建、改建的房屋建筑工程（包括與其 配套的線路管道和設備安裝工程、裝飾工程）、市政基礎設施工程和古建園林工程 均

2、按本通知要求執(zhí)行。第二條 現(xiàn)場安全文明施工措施費是指工程施工期間為滿足安全生產、文明施 工、職工健康生活所發(fā)生的費用。內容詳見附件一?，F(xiàn)場安全文明施工措施費分為基本費、現(xiàn)場考評費和獎勵費三部分?；举M為施工單位在施工過程中必須發(fā)生的安全文明措施的基本保障費用。 現(xiàn)場考評費是指施工單位執(zhí)行有關安全文明施工規(guī)定，經考評組織現(xiàn)場核查打 分和動態(tài)評價獲取的安全文明措施增加費。獎勵費是指施工單位加大投入，加強管理，創(chuàng)建省、蘇州市級及我市文明工地 的獎勵費。第三條 現(xiàn)場安全文明施工措施費以分部分項工程費為計費基礎，結算時按審 定的分部分項工程工程費進行調整。具體費率標準見下表 .現(xiàn)場安全文明施工措施費費率

14、條 對安全防護、文明施工有特殊要求和危險性較大的工程，需增加現(xiàn)場 安全文明施工措施及方案論證、審查等費用的，由施工單位在措施費中單獨計取。第六條 為提高我市建設工程施工現(xiàn)場管理水平，保證現(xiàn)場安全文明施工措施 費落實到位，決定成立張家港市建設工程安全文明施工措施費現(xiàn)場考評指導小組 由局建筑業(yè)科、建筑業(yè)管理處、安監(jiān)站、招標辦、質監(jiān)站等單位組成，負責現(xiàn)場考 評、爭議裁定和舉報查處，并對已進行現(xiàn)場考評的項目進行抽查 . 現(xiàn)場考評指導小 組組成名單見附件四。第七條 申請考評時間 : 土建工程為主體結構封頂前約一星期，其他工程完成工 程量 70左右時，由施工單位向安監(jiān)站提出書面申請（建設工程現(xiàn)場安全文明

15、措施費考評申請表建筑工程現(xiàn)場安全文明施工措施費考評標準表見附件 二） ，由安監(jiān)站負責組織現(xiàn)場考評，未申請考評的工程不得計取現(xiàn)場考評費及獎勵 費.第八條 現(xiàn)場考評費率實行浮動費率 , 根據(jù)現(xiàn)場考評平均分與綜合評價修正比率 確定。現(xiàn)場考評平均分為安全綜合考評、安全專項檢查等各類檢查得分和考評指導 小組檢查得分的平均值。項目竣工后 , 由考評指導小組根據(jù)整個施工過程現(xiàn)場管理 狀況作出綜合評價。綜合評價分好、較好、一般、較差、不合格，修正率分別為 100%、95、90、80%、0. 具體計算方法：現(xiàn)場考評費率=現(xiàn)場考評費率標準值X綜合評價修正比率綜合評價結果對應取值見下表 :現(xiàn)場考評level of

22、管理部門確 定的費率計算現(xiàn)場安全文明施工措施費。第十條 我市城鎮(zhèn)區(qū)域內的拆除工程，其現(xiàn)場安全文明施工措施費參照機械施 工大型土石方工程執(zhí)行，基本費率 1，現(xiàn)場考評費率 0。 6，計費基礎為修繕定 額的直接費加綜合間接費 .第十一條 建設單位與施工單位應當在施工合同中明確現(xiàn)場安全文明施工措施 費用，以及費用支付計劃、使用要求、調整方式等條款。實行項目總承包工程，總承包單位依法將建筑工程分包給其他單位的, 總承包單位與分包單位應當在分包合同中明確現(xiàn)場安全文明施工措施費用，以及費用支付 level of security personnel on duty, fire security measur

28、現(xiàn)場安全文明施工措施費?？顚Ｓ?，在財務管理 中單獨列出現(xiàn)場安全文明施工措施項目費用清單備查 .工程總承包單位對建設工程安全文明施工措施費用的使用負總責 . 總承包單位 應當按照規(guī)定及合同約定及時向分包單位支付現(xiàn)場安全文明施工措施費?？偝邪鼏?位不按規(guī)定和合同約定支付費用，造成分包單位不能及時落實安全防護措施導致發(fā) 生事故的，由總承包單位負主要責任 .第十四條招標人或招標代理機構不按規(guī)定在招標文件中單獨計取現(xiàn)場安全文明 施工措施費或費用取費標準不符合規(guī)定的，由招投標管理機構責令限期改正；逾期 未改正的 , 對招標文件不予備案，并給予通報批評。施工單位在投標報價時，未按規(guī)定單列現(xiàn)場安全文明施工費用

29、，或計取費用低 于現(xiàn)行規(guī)定標準的，不得推薦為中標候選人第十五條 建設單位不按規(guī)定支付現(xiàn)場安全文明施工措施費的，由縣級以上建設行政主管部門依據(jù)建設工程安全生產管理條例第五十四條規(guī)定，責令限期改正；逾期未改正的，責令該工程停止施工 ; 造成事故的，應承擔相應的法律責任 . 第十六條 施工單位挪用現(xiàn)場安全文明施工措施費或安全生產、文明施工措施 不落實的 , 由縣級以上建設行政主管部門依據(jù)建設工程安全生產管理條例第六 十三條規(guī)定，責令限期改正；逾期未改正的，責令停止施工，并按有關規(guī)定處以罰 款; 造成事故的，按有關規(guī)定追究其法律責任，并將其行為列入不良檔案記錄.第十七條 本通知自 2006年3月1日起
